Job Description
The Home Visitor With the Home vis,tlng Program will provide developmental Information, support and encouragement to Jn<Jlvlduals and ramnles with a primary focus on pregnant women, Infants, young children and expectant lathers; perform case management services as directed utlllzlng the Parents As Teachers home vlsttabon model; case management services wlll indude client mtake, data collection and inpuL racilltalion and development or service plans including Junctions for advocacy, crisis management. referrals and diredmg families to local resources for health care. substance abuse, menlal health, housing, food, joD training programs and Insurance providers. This position works under the administrative supervision or the Home Visiting Program Lead.
Starting Salary· $17.10
KEY ACCOUNTABILITIES:
- Provides the coordination and delivery of services to enable health and well-adjusted childhood development.
- Identify and enroll Native American families with children under the age of five into the program.
- Assists in the planning and facilitation of group activities and meetings that focus on child development, parent-child interaction, and education on family well-being.
- Documents and keeps records of all visits and case management services provided to families.
- Supports the organization as needed.
Qualifications
QUALIFICATION$/EXPERIENCE/REQUIREMENTS:
- Must possess a Bachelor's degree in early childhood developmenVeducation, social work, public heath, or related discipline appropriate to tllis position.
- Must possess two years of specialized experience working with young cllildren and/or parents, preferably within the American Indian/Alaskan Native community.
- Must possess a valid and unrestricted Arizona driver's license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver's license during employment.
- Must be insurable under NATIVE HEALTH's liability auto policy.
- Must pass a substance abuse test upon employment and submit to random testing during the course of employment.
- Must have an update Immunization (IZ) Record.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon emloyment and provide an annual update.
- Must obtain a First Aid/CPR Card within (op) calendar days of employment and maintain a valid CPR Card during employment.
